How do I know if I am eligible for one of
Asuris Northwest Health's medical plans?
To be eligible to apply for our individual preferred
plans, you must reside in our service area for
at least 30 days prior to submitting your application
and continue to live in our service area for six
months of each year. Eligible dependents include
your spouse and/or children under the age of 23.
The service area includes the Washington State
Counties (Adams, Benton, Chelan, Douglas, Ferry,
Franklin, Grant, Lincoln, Okanogan, Pend Oreille,
Spokane & Stevens).

covered at 100% after the copay (before the
deductible)**.
- Drugs: Generic prescription drugs paid
with a copay (before meeting the deductible).
The RegenceRx Discount Program is also
available.
- Preventive Care Benefits: Available
with Breakthru 70 and Breakthru 80.
- Asuris Advantages: Discounts on LASIK
surgery, hearing aids, contact lenses, and other
services.
*X-rays, lab services and other professional
services are subject to the deductible and coinsurance.

Comprehensive Preferred Plan
Our comprehensive plan is designed to give you
a high level of protection for most major and
minor medical expenses, including prescription
drugs. Benefits include:
- 100% preventive care coverage with
no deductible, up to the plan maximum. Preventive
care benefits include routine physicals and
regular check-ups.
- Prescription drug coverage without
a deductible to help keep your costs down.
- $750 deductible
